For people with heart failure and other chronic conditions, taking medications as prescribed is usually the largest single determinant in maintaining good health. Embrace's pharmacy services are designed to ensure that the right medications are taken at the right time.
Embrace consolidates prescriptions from all of your loved one's doctors and creates a complete medication list which is then shared among all caregivers. This ensures that all are up to date and working together.
Our pharmacists are available for consultation, and they work to substitute less costly generic drugs when available and see if there are opportunities to remove drugs. Because our pharmacists work exclusively with older adults, we have a deep understanding of what typically works best for your loved one's specific set of conditions.
Your nurse practitioner reviews medications with your loved one to ensure that the drugs are being taken properly. If side effects are a problem, your nurse can work with the pharmacist to identify possible alternatives and recommend these to the prescribing physician.

Medicare covers these additional services for older adults with multiple chronic conditions because they have been proven to improve health outcomes. Many caregivers are unaware that these services are available and paid for by Medicare.
Although Medicare covers the majority of the costs for your Embrace nurse, there may be a small copay, usually less than $10.00 per month. In many instances the copays are covered by Medicare Advantage or a supplemental insurance plan. If you have questions about costs, don't hesitate to call, we'll make sure there are no surprises.
Medicare is willing to pay for these services because the regular interactions with a nurse and monitoring have been proven to improve health and reduce hospital and emergency room visits.

Even though much of the cost of a hospital stay is picked up by Medicare, out-of-pocket costs could be thousands of dollars. If your loved one is admitted to the hospital on an observational basis, and then discharged to a care facility, those costs can rise to almost $5,000.
When you use Embrace's Medicare covered services, you are required to make a monthly copay, although supplemental insurance may cover it. In most instances, this payment is less than $10 per month or about $100 per year, less than a tenth of the cost of the least expensive hospital admission.
After most hospital stays, older adults' overall health never quite recovers to the point it was before the event that caused the hospital stay. Often what happens is a recurring cycle of declining health and hospital stays until the hospital discharge is to a long-term care facility which the individual never leaves.
